Am I reading this wrong or can Midori both be rezzed and use her ability in step 2.3, when the runner has already decided whether to jack out?

 

Also, can't she be combined with Amazon Industrial Grid? To reduce the cost of the ice being swapped in by 3? (Since I probably want to rez it right away anyhow?)

For the second point: yes, go ahead. For the first, a triggered ability such as Midori's has to be used as soon as its trigger is met. That is at point 2 in the run timing chart. On the other hand, the earlist opportunity to rez Midori after a run is initiated is at 2.3.

Ok, since the ice is actually being installed on the server being run on, can I then trash any cards already installed in that server?  Maybe an agenda that will be scored when they complete the run…

You can only trash ice when you're installing ice.  You can only trash stuff IN a server (as opposed to protecting it) when you install something else IN the server.

So she has to be rezzed before the run starts, but if I use her to swap the outermost Ice on the server, the runner has to face whatever I swap in if I rez it (because now its the outermost Ice)!

Absolutely terrifying card. Just installing her and rezzing her (as long as there's any ice on the server at all) means I'm not going to be running that server at all unless I have a ton of money, the corp has no money, or I've got an Inside Job.

Being a fan of the show, it's just so interesting to see Tyrell and Gauis having a back and forth. (and yes, I'm aware the spelling probably isn't right for "Chief's" name, but I don't feel like looking it up atm)

It is not entirely clear to me whether she has to be rezzed before the run starts.  At first glance it seems her ability has to trigger on 2 (not 2.3, the first opportunity to rez cards) which would mean you would need to rez her before the run for the first ICE, or during the previous encounter for later ICE.  The subtopic layout of the run diagram isn't crystal clear on this though, as it could be interpreted that all of step 2 (including 2.3) counts as "when the runner approaches" meaning you could spring it after the decision to jack out.  Oh, and Inside Job won't help overly much, as it doesn't have to be the first ICE Midori swaps, it could easily be the second (though again, depending on timing you might be able to jack out after the switch but before the rez).  We'll have to wait for the next FAQ or a ruling, I guess.
