Remote Server … Do Upgrades and Agendas(or Assets) get placed in the same area?

 When creating remote servers, should it be obvious to the runner where Upgrades are located, and where Agenda/Assets are located?

If I have two cards behind ICE on a remote server, should the Runner be able to tell if it is 1 upgrade and 1 Agenda (or Asset), or 2 upgrades with no Agenda (or Asset)?

And for that matter, if I have only 1 card behind ICE, should it be obvious to the runner that it is either an agenda/asset or upgrade?

No it should not be obvious. If there is two all that the runner knows is that at least one of them is an upgrade.

 In general, it will become obvious if one of the two starts getting advancement tokens.
