 In scenario 1 the keeper has an action card at his disposal which summons a maniac in the space of a lone investigator.An investigator is considered lone when no other investigator is in his room or in his space?

Also,is it possible for a monster to hold two sample tokens and deliver them in the same turn to the altar with a single use of the action card: Take Sample?

Lone survivor: This is unclear, you're right. Feel free to send a message to FFG. I played it as "alone in the space", which was... well... most of the time, really. They tried to stick together, but eventually failed of course. There is, however, no rule that says that I'm right.

Place two sample tokens at the same time: Yes, a monster can carry and put down to sample tokens at the same time.

With the same use of "Take Sample": You don't have to use "Take Sample"spend any Threat to place a token on an altar. It's just "there", the ability to do so, for free. It's like a general rule, stating "If a monster with a token starts its turn at an altar, the Keeper can place the token there." Totally unrelated to the Action Card, effectively.
Using the Keeper action card "Take Sample" really is there only to take a sample from an investigator, which costs 1 Threat.
