Clarification on Multiple attacks and active actions

There seems to be some debate among me and another gamer regarding multiple attacks and active actions.  It seems to suggest that ANY kind of attack drains an active action, and gets increasingly confusing when you apply it to multiple attacks from high attack ability (like at 100, 200, etc)  Does each attack drain an active action? Do they get penalties from using multiple active actions if used on the first attack?( -25 for first attack, -75 for second )  Does say attacking with an offhand drain an additional active action as well?

This has got us stumped so any feedback/ideas would be great!  Thanks a bunch!
