 I'm a little confused on creatures and cards. Do all creatures get the basic cards like Melee strike and dodge? If a card in the crreature vault says its used by orcs do all orcs get the card or is it only when gm wants to spice it up?

Yes, all creatures get the same basic actions PC's get, if they meet prerequisites (e.g., animals won't be Blocking with a shield, an AG 2 monster won't have Dodge).  So goblins with bows can used Ranged Attack even if they have no "special" ranged actions.

This is stated in the Tome of Adventure (soft book from core set) on page 47, the Creatures and NPC Actions section in colour.    It is stated in the hardback Creature Guide on page 8 under Custom Creature Actions.

The Creature Guide hardback is a bit of a mess (duplicate information between the two times each creature described and no standard array of actions as in the Tome and adventures).  Generally what the Tome of Adventure and other adventures show is that most creatures have 1-3 special actions as well as the basic ones.  You wouldn't know this using the Creature Guide whose text suggests only creatures with action icons on their cards get extra actions (a Gor has no icon, a Wargor has 1). 

The Tome of Advanture page 44 and hardback GM Guide is a better advice for additional creature actions, page 44 (TOA) 46 (GM Guide) say "in general addtional action cards = Expertise +1"  A Gor is Expertise1 and Wargor 2 so that actually works out to match the published Tome of Adventure versions.  This math doesn't hold true everywhere but is a good guide in my view.

E.G. an Ungor has only Savage Strike, a Gor has 2 more than that, a Wargor 3 more than that.

To expand on Valvorik's very accurate answer, the general answer is "it depends."

Yes, they all get the basic actions they qualify for, buy other than that it depends. Actions cards are a basic tool to scale an encounter to your party. Have you got a high toughness PC who needs to be taken down a notch? Find one of the action cards that reduces damage but increases critical wounds. Stuff like that. It's also a great way to mix things up and personalize enemies without upgrading them to full nemisis level. For instance, my players just took on an orc with a whip. Since a whip is a ranged weapon I gave him things like Knock Back Shot. It kept the players on their toes.

The traits on the cards, like almost all traits, are suggestions as to who can use them, not limitations. If you can think of a reason why this particular troll would have an action best currently represented by a card with the Skaven trait, go for it.
